CHECKLIST

  1. Original Birth certificates (long version with parents names) plus three photocopies.
  2. Three photocopies of your signed passports.
  3. One original statutory declaration done according to our sample, that was signed by you in front of a Lawyer (Notary Public) and that is signed and sealed by that same Notary.
  4. Two photocopies of the signed original statutory declaration.
  5. Only if applicable, Original divorce or death certificates plus three photocopies.
  6. Pre-paid Express Post or courier (FedEx, UPS, DHL, Priority) envelope with your address and phone number for us to return your documents. (Purolator not accepted)
  7. Two separate money orders as per our calculation of fees page.
  8. This filled in form.
