CHECKLIST

  • Three photocopies of your birth certificates.

  • Three photocopies of your signed passports.

  • One original statutory declaration done according to our sample, that was signed by you in front of a Lawyer (Notary Public) and that is signed and sealed by that same Notary.

  • Two photocopies of the signed original statutory declaration.

  • Only if applicable, Three photocopies of divorce or death certificates.

  • Pre-paid Express Post or courier (FedEx, UPS, DHL, Priority) envelope with your address and phone number for us to return your documents. (Purolator not accepted)

  • Two separate money orders as per our calculation of fees page.

  • This filled in form.
